I’ve never done a prep like this. The patient had subgingival distal caries on 19 but the occlusal surface was completely caries free. He was also missing 18. Because of this, my attending recommended that I access JUST the distal aspect of the tooth, to spare the unnecessary removal of occlusal tooth structure that would occur if I dropped a box from the distal occlusal. Trying to angle the bur all the way back there got super awkward, and I ended up using the excavator for majority of the caries removal. Once I removed all the caries, I applied conditioner. Then drilled a small hole in the tofflemire matrix so that when I put it on, the hole would correspond to where I had my prep. Once I had the matrix in place, I had to make sure the glass ionomer tip, was inside the hole in the matrix so that my prep could be filled. WHEW. Would you have just done a DO anyways even though the occlusal surface was caries free and didn’t have an adjacent tooth to the distal? Is there something else that could’ve been done?
